Application for Inscription to the Book of Remembrance

Please ensure that your inscription us clearly written in the correct boxes below. Do not forget to include the spaces in your inscription. A space counts as a letter or number. Salisbury City Council reserves the right to refuse an entry which is considered unsuitable. Please refer to the fees and charges sheet to confirm current costs.

Please allow a minimum of six months to return your application form to us before the intended entry date. This allows the Book of Remembrance to be updated.
